Optimizing Prophylactic Anticoagulation in Burns is Associated with Low Incidence of Venous Thromboembolic Complications

Venous thromboembolism (VTE) complications in burn patients are often under-diagnosed and potentially serious. Thromboprophylaxis this population remains controversial. Objective: Assess the impact of optimizing prophylactic anticoagulation with enoxaparin on incidence venous (VTE). Methods: Case-control study conducted intensive care department Tunisia during 24 months, (February 2018- February 2020). Patients were divided into 2 groups according to modalities: - G1 (Equation) receiving enoxaparine en mg/12H = 22.8 + (3.3 × % TBSA/10) (1.89 (weight kg)/10)) G2 (No équation) at a dose 0.5mg/kg, twice daily The goal antifactor Xa level was 0.2- 0.4 IU/ml Results: During period, 216 included groups: (n= 108) et 108). comparable terms sex, age, weight, burned skin surface VTE risk. No episodes bleeding, thrombocytopenia, or heparin allergy documented either group. higher group than 1 (8.3% versus 3.7%; p=0.001 an OR=1.6 CI [0.47-1.03]). length stay longer for significant difference (30 days vs 22 days; p=0.001). Mortality same two groups. Conclusion: Optimizing thromboprophylaxis severely enoxaparin, using dosing eqauation allows achieve anti-Xa reduce risk complications.
